Currently, commonly used infusion flexible packaging materials are polyethylene (PE), polypropylene (PP), polyvinyl chloride (PVC) and ethylene-2-vinyl-acetate-copolymer (EVA).
PVC infusion bag, compared with other materials, has some obvious advantages, for example, lighter weight, more flexible, durable and watertight, lower cost and easier transportation, etc. coupled with the clinical use of flexible packaging infusion, without access to the air, to avoid the infusion of liquid pollution, is conducive to ensuring the quality of infusion, to reduce the infusion reaction. These advantages make PVC swiftly become an important component in the field of infusion packaging.
As the largest PVC infusion bag manufacturer in China, Ruide Medical has been studying the material properties, manufacturing process and performance improvement of PVC infusion bags, with the aim to produce China and even the world's best PVC infusion bags.
This article focuses on the intricacies of PVC infusion bags in medical use. With the combination of Ruide Tech team's experimental results and manufacturing experiences, targeting on 1. drug adsorption, 2. migration of PVC residues, 3. ventilation and moisture permeability of PVC infusion bag, to describe the safety problem of PVC infusion bag, and put forward the corresponding improvement method accordingly.
1. Drug adsorption
PVC is a polar plastic material, adsorption of many drugs, especially for high lipophilic non-ionic compounds adsorption is relatively considerable. The conclution is, for some drugs, adsorption will increase with temperature. This kind of adsorption, will reduce the number of active ingredients of drugs, leading to prescription medication failed, reducing the efficacy of drugs and increasing treatment costs. Therefore, the study of PVC infusion bag on the adsorption of different drugs is of great significance.
Insulin is a common drug for the treatment of diabetes and consumptive diseases. Insulin has a significant adsorption with PVC infusion bag. We studied the effect of PVC infusion bags on insulin at room temperature. We added 40 μl of insulin to the PVC bag and PP bottle containing 250 mL of 0.9% sodium oxide solution respectively. After shaking, the samples were collected and the insulin concentration was determined by high-performance liquid chromatography (HPLC). The results showed that the adsorption rate of insulin in the PVC infusion bag reached 63.11% after 12h, while the PP bottle had no obvious adsorption effect on insulin. We also studied the adsorption of insulin on PVC infusion bags. We will be 200μg insulin injection into 500mL 5% glucose injection of PVC bag, the infusion pump to control the drip rate, charge the different time period of the liquid, the determination of liquid insulin content. The results showed that the adsorption equilibrium was reached at about 30 min, and the average adsorption rate was 31.25%.
In other experiments, we also found that some other drugs, such as enoxacin, norfloxacin, fleroxacin, gatifloxacin, ciprofloxacin and other fluoroquinolones have significant adsorption. According to the relevant literature and information, PVC infusion bag also on chlorpromazine (antipsychotics), amiodarone (antiarrhythmic drugs), carmustine, on tinidazole, Zedoary turmeric oil, macrocyclic piperazine (FK250 ), Urokinase, phentolamine, atropine, cefotaxime sodium and other drugs have different degrees of adsorption.
Therefore, PVC infusion bag has a significant adsorption effect with some drugs. It could reduce the actual concentration of injection drugs, and reduce the medical efficiency of treatment.
